What Are Dermal Fillers?

Dermal filler is a gel-like substance that is injected under your skin to restore volume, soften lines and wrinkles, as well as to enhance your facial shape. It is also known as “liquid face lift”. As you get older lose subcutaneous fat in your face. This makes your smile lines and crow’s feet become more visible as your facial muscles are operating closer to your skin’s surface. This loss of facial volume is exacerbated by the stretching of your facial skin. Sun exposure, heredity, and lifestyle are among the factors that also influence your facial skin.

Dermatologists can now use a variety of fillers to reduce the appearance of wrinkles on your face, lips, and hands. Fillers can be used to:

  • Restore fullness to your cheekbones, under-eye hollows (tear trough), and deep wrinkles.
  • Diminishing scars.
  • Removing fine lines.
  • Chin augmentation.
  • Nose augmentation.
  • Jaw augmentation.
  • Plump lips.
  • Smile lift.
  • Eyebrow lift.
  • Hands rejuvenation.
  • Reconstruct contour deformities in your face.

Recovery time varies depending on the patient and the type of filler injected. Most activities can be resumed immediately, but it is generally recommended that you avoid strenuous physical activity for the first 24-48 hours to reduce swelling and bruising. When your own fat is used as the injected filler, the healing time is extended and may take several weeks. Following treatment with any dermal filler, you may notice the following changes in your appearance:

  • Treated areas have an “overfilled” appearance
  • Swelling or bruising of varying severity
  • Numbness or redness for a short period of time
  • At the injection sites, there may be palpable lumps or hard areas.
  • Hives and swelling ( symptoms of a hypersensitivity/allergic reaction)

Most conditions can be relieved with topical icing and massage and will improve in a matter of hours or days. When accompanied by pain or weakness on one side of your body, it is critical that you contact your surgeon and seek treatment as soon as possible.

Facial fat grafting is a surgical procedure with a unique treatment protocol. Fat grafting is typically performed as an outpatient procedure under general anesthesia or local anesthesia with sedation. Depending on the extent of the liposuction, up to two weeks of recovery time may be required. During your consultation, your cosmetic surgeon will go over what to expect from surgery and recovery.

The area will be cleaned just before the treatment, and you may be given a topical anesthetic to numb the area prior to injection. Many filler products also contain lidocaine, a mild anesthetic that is meant to reduce discomfort during and after your treatment. Your doctor will then strategically inject a precise amount of filler beneath the skin.

You should be able to see results immediately after filler injections, depending on the product and the areas treated. Mild bruising and swelling may occur in some patients, but these are transient and should go away within a few days of treatment.

You will be able to resume your normal activities immediately following treatment, but your cosmetic surgeon may request that you refrain from exercising or engaging in strenuous activity for the day.

Your cosmetic surgeon will evaluate your area of concern and review your medical history during your consultation. While the risks of dermal fillers are low, you must fully disclose your medical history prior to treatment because certain allergies, skin and neurological conditions, or medications can jeopardize your safety or results. For example, if you have been taking certain painkillers or blood thinners, you must inform your cosmetic surgeon as these medicines increase the risk of bleeding.

Dermal filler injections are non-surgical procedures that are typically performed during an office visit. The first step in your treatment will be a consultation with your cosmetic surgeon to discuss your concerns and goals.

Your dermatologist can assist you in determining the cause of your skin problem and can also help in assessing if a dermal filler is right for you or not. Your dermatologist can then develop a treatment plan that is right for you. That one visit could make a significant difference in your quality of life for years to come. However, given the delicate nature of the procedure, it’s best to work with a board-certified dermatologist that can ensure the process is safe and effective.

If you and your dermatologist agree that procedure is the best choice for you, your dermatologist will devise a tailor-made treatment plan exclusively for you. Your dermatologist will consider a variety of factors when developing this plan, including your main cause of skin problem, location, severity, etc. If you have taken any kind of previous skin-related treatment, be sure to tell your dermatologist about that before the treatment begins.

With so many dermal filler products on the market, it can be difficult to know which option is best for you without the assistance of a skilled cosmetic surgeon. Because each product is uniquely formulated to have a specific texture, density, and injection depth, certain fillers work better for specific areas of concern. While your doctor will determine which product is best for you, the diagram below shows where cosmetic surgeons typically apply certain products:

Cosmetic surgeons use a variety of FDA-approved filler products. In general, fillers are classified according to the substance from which they are made. They can be:

  • Hyaluronic Acid (HA)
    • Hyaluronic acid is a naturally occurring substance found in your skin. It aids in keeping your skin plump and hydrated. HA fillers are typically soft and gel-like in texture.
    • The effects last for around 6-12 months or maybe longer in your body. After that, the filler particles are gradually and naturally absorbed by your body. Most HA fillers contain lidocaine, which helps to reduce discomfort during and after treatment.
    • Although side effects are uncommon, they can include redness, swelling, and bruising at the injection site. The filler may also appear as tiny bumps under the skin. This is a problem that frequently improves with time.
  • Calcium Hydroxylapatite (CaHA)
    • Calcium hydroxylapatite is a naturally occurring substance that is mostly found in your bones. When used in a filler, the calcium particles are nearly microscopic and suspended in a smooth gel.
    • A CaHA filler has a thicker consistency than a hyaluronic acid filler and typically lasts longer, about 12 months for most patients. Calcium hydroxylapatite, which is typically used for deeper lines and wrinkles, is also reported to help stimulate natural collagen production.
    • This type of dermal filler is known for producing a very natural result, does not b, and has very few side effects.
  • Poly-L-lactic Acid
    • Poly-L-lactic acid is a synthetic substance that is biocompatible i.e. it is safe to use in your body. It is also biodegradable. For many years, it has been used in medical devices such as dissolvable stitches.
    • Poly-L-lactic acid threads are also known as “collagen stimulators,” as they help in the production of natural collagen which is important for smoothing fine lines. It is typically used to treat deep wrinkles on your face, with results lasting up to two years.
    • It does not yield immediate results. Instead, it stimulates your body’s own collagen production, so results appear gradually over a few months. While this type of dermal filler is considered semi-permanent, you may still require touch-ups on a regular basis.
  • Polymethylmethacrylate (PMMA)
    • Polymethylmethacrylate (PMMA) is a biocompatible synthetic substance that has been used in medicine for a long period of time. PMMA is used as dermal fillers in the form of a “microsphere” or tiny ball that remains under your skin for a long time to provide support.
    • Collagen, a naturally occurring substance in your skin that provides structure and firmness, will also be present in PMMA fillers.
    • PMMA is frequently used instead of collagen replacement therapy or hyaluronic therapy when a more permanent solution to facial wrinkles is desired.
    • One disadvantage of PMMA is that it requires multiple injections to create volume, and it can take up to three months to see the full effects. It could also be visible beneath your skin. To avoid any unfavorable outcomes, it is critical that your plastic surgeon is well-versed in the proper technique, therefore choose a board-certified surgeon for getting maximum benefits.
  • Polyalkylimide
    • Polyalkylimide is a semi-permanent dermal filler that is frequently used by plastic surgeons to treat deeper wrinkles like nasolabial folds or depressed scars, enhance your cheekbones and jawline, and replace facial volume loss with age.
    • It is biocompatible, and no allergy test is required for this filler. It is radiotransparent, which means it will not interfere with x-rays.
    • Over the course of about a month, a thin layer of collagen forms around the injection. Eventually, the gel is completely encircled. A large volume can be injected in a single procedure. This product, which is thought to be quite stable over time, can even be removed if required.
  • Collagen Wrinkle Fillers
    • These wrinkle fillers were derived from animals, and they also had a higher rate of an allergic reaction, necessitating allergy testing beforehand.
    • New methods of processing collagen have helped to reduce the risks, making these injections safer and more useful to a broader range of people. Although the results do not last as long as other wrinkle fillers, many people believe they look more natural.
    • Side effects of collagen filler injections include bruising and redness at the injection site, as well as a risk of an allergic reaction.
  • Autologous Fat Injections (Facial Fat Grafting)
    • Autologous fat injections are the only injectable filler treatment that requires a minor surgical procedure, but the results are long-lasting.
    • Liposuction is typically used to extract your own fat from another area (autologous means “from the same person”).
    • Following that, the fat is purified and injected into the face to help restore volume to the cheeks, temples, lower eyelids, and other areas.
    • Fat injections necessitate specialized training to be performed safely and effectively, and should only be performed by an experienced, board-certified cosmetic surgeon. The results can be semi-permanent, though a series of injections may be required over time.
  • Platelet-rich Plasma Injections
    • Another type of autologous wrinkle filler/volumizer is platelet-rich plasma injection (also known as “vampire lift”). Blood is drawn from the arm, treated, and then injected into the patient’s face. The effects can last between 12 and 18 months.
    • The risks are similar to those associated with other wrinkle fillers, including bruising, redness, and swelling at the injection site. These injections do not require an allergy test because the fillers are derived from your body.
  • Liquid Silicone Oil Injection (Synthetic Filler)
    • It is a permanent, less expensive, and biocompatible type of filler that has minimal side effects when correctly injected. It gives a natural feel when injected into the deeper folds. The final result is not seen right away and may require multiple visits.
    • The single most important issue is that the silicone is delivered in extremely small microdroplets. It involves placing tiny “seeds” of silicone in 0.01-mL aliquots.
    • Silicone oil stimulates the production of collagen in your body. When the microdroplets are injected into your body, they become encapsulated by collagen because they are recognized as a foreign body. This process takes about 4 weeks.
    • This filler may result in side effects such as redness, swelling, or bruising at the injection site. Other, rare, side effects include nodules or bumps under the skin that can be seen and felt and, in rare cases, may require surgery to remove.

Dermal fillers’ long-term effects are determined by the product, the area of treatment, and your condition. Although this is not a hard and fast rule, the denser the product and the deeper it is injected, the longer it will last. Your cosmetic surgeon will simply repeat treatment to maintain your results by adjusting the amount and techniques as needed to ensure optimal results.

Because hyaluronic acid fillers are the most temporary option, they are frequently recommended for first-time filler patients. These typically last between 6 and 18 months. Lip injections will last a little longer than nasolabial fold injections. Certain HA fillers, such as VOLUMA, are designed to last longer, but they are typically restricted to specific areas, such as the cheeks.

Because synthetic fillers are not absorbed by the body, they last longer. They can be a great option for the right patient, but you should be prepared to commit to long-term results—and choose an experienced, qualified provider whose aesthetic style you like.

Fat injections are intended to be permanent; however, some of the injected fat is not expected to survive. Your cosmetic surgeon may initially overfill the treatment area, resulting in a fuller appearance that will gradually settle into a more natural appearance.

Severe dermal filler complications are uncommon. Potential risks vary depending on the specific filler used and the relative permanence of the filler substance, but they include the following:

  • Skin eruptions resembling acne
  • Asymmetry
  • Bruising at the injection site
  • Bruising
  • Skin damage that results in a wound and possibly scarring
  • Infection at the site of injection
  • Lumps
  • The filler’s palpability beneath the skin’s surface
  • Necrosis of the skin (ulceration or loss of skin from disruption of blood flow)
  • Itchy rash on the skin
  • Skin rashes
  • Swelling
  • Wrinkle under– or over-correction

In extremely rare cases, the filler may be injected into your blood vessels rather than under your skin. This might block your blood flow. If filler migrates into your eyes or other parts of your body, then it may harm them as well.

Because dermal filler injections can cause such complications, it is critical to be under the care of a board-certified plastic surgeon that understands the risks and is trained and prepared to deal with any complications that may arise.

The cost of getting dermal fillers depends on the type and severity of the skin condition you have and if additionally PRP is needed for the faster and better healing of the skin condition. Single session cost can vary from Rs.15,000 to 30,000 per syringe, depending on what is being treated.
